Clinician Master Agreement

01What this agreement is
This is the agreement between you (the clinician) and GlideHire. It covers how GlideHire introduces you to facilities, how the reporting bonus works, and your responsibilities when you use the platform. It is short on purpose. If anything is unclear, email gabrielfarkas86@gmail.com.
02Who you are
You are a licensed anesthesia clinician — an anesthesiologist (MD/DO), CRNA, or CAA — verified against the NPPES registry. You are signing this agreement under your own name. You are not an employee, agent, or contractor of GlideHire; this agreement does not create any employment relationship between us.
03How introductions work
When you apply to a job, message a facility, or are viewed by a facility on GlideHire, we record this as an attribution event — a timestamped record that the facility met you through GlideHire. You consent to GlideHire maintaining this record and using it to determine reporting-bonus eligibility (§4) and platform anti-fraud monitoring.
You agree not to coordinate with a facility to evade the attribution system — for example, by asking the facility to pretend you never interacted on GlideHire so they can avoid the placement fee. Doing so forfeits any bonus and may result in account termination.
04Reporting bonus
If a facility hires you within twelve (12) months of your most recent attribution event, GlideHire pays a reporting bonus to you:
- Per-diem / 1099 / locum hire: $500
- Part-time permanent hire: $1,500
- Full-time CRNA hire: $2,500
- Full-time MD/DO hire: $3,500
Amounts are provisional pending counsel review and may be adjusted before the agreement is finalized. Once finalized, the amount in effect at the time of your hire applies.
The bonus is paid only after (a) the facility pays their placement-fee invoice in full, AND (b) you have remained employed by the facility for at least 90 days from your start date. If either condition fails, the bonus is forfeited.
You can self-report a hire at any time via the “Did you start a new job?” prompt on your dashboard or at /clinician/report-hire. A self-reported hire triggers admin review.
05Tax treatment (1099-NEC / independent contractor)
The reporting bonus is paid to you as an independent contractor — it is not wages, salary, employee compensation, or a referral fee from your employer. GlideHire does not withhold taxes; you are responsible for reporting bonus income on your personal tax return.
When your lifetime bonus earnings from GlideHire reach $600, we will ask you to complete a Form W-9 before we can pay further bonuses. At year-end GlideHire files a Form 1099-NEC with the IRS reporting your total bonus payments and sends you a copy.
06Communications consent
You consent to receive transactional emails about your account, applications, messages, hires, and bonuses. You may also opt in to marketing communications (saved-search digests, market-rate updates, product announcements). One-click unsubscribe is provided per message; transactional email cannot be unsubscribed without closing your account.
If you provided a phone number for SMS verification, you consent to receive transactional SMS related to verification and critical account-security events. Standard message and data rates apply.
07Profile and compensation data
Your profile is private by default and visible only to facilities you interact with through GlideHire. GlideHire does not publish your name, NPI, or contact information without your action.
If you contribute to the compensation self-report survey, your responses are stored anonymously — your survey rows are never linked to your facility or employer by name. Aggregate compensation statistics are published only where the underlying sample is large enough to prevent re-identification (n ≥ 5 in product, n ≥ 10 publicly).
08Your responsibilities
You agree to:
- Use only your own NPI and your own name on the platform.
- Not impersonate another clinician, share your credentials, or let anyone else use your account.
- Keep your profile information accurate.
- Not scrape the platform or use it for purposes outside connecting with facilities.
- Not upload another person’s protected health information (PHI).
09Termination
You may close your account at any time. GlideHire may terminate or suspend your account for breach of this agreement, fraudulent activity, or misuse. On termination, your profile is hidden from facilities; pending bonuses tied to active attributions are paid out if their conditions are met within the existing verification window.
10Dispute resolution
We try to resolve disputes informally first. Email gabrielfarkas86@gmail.com before initiating any formal action. If informal resolution fails, disputes are governed by Pennsylvania law and resolved in the courts of Philadelphia County, PA. Counsel may revise this to require arbitration; pending review.
11Changes
We may update this agreement. If we do, you will be asked to re-sign before continuing to use protected features. Material changes — changes to the bonus amounts, the attribution model, or your tax obligations — require an explicit re-sign; cosmetic edits do not.
12Acknowledgment
By signing this agreement you confirm you have read it, you are signing under your own name, and you agree to its terms. The signing flow captures your typed name, IP address, browser user agent, and a timestamp as evidence of your acceptance.
